Request a case report

Through this service, you can request to obtain a patient's case report from New Life Rehabilitation Centre.

Process Steps

  1. Submit the request via email.

  2. The requester must present a valid ID card upon receiving the report.

  3. Receive the report by personally visiting the centre and presenting a valid ID card.

Please note: the service duration to obtain the report is 3 business days.
